Extraction of judgment teeth (included teeth)

In dentistry, teeth are referred to as included teeth when they fail to erupt and thus remain internally in the bone, or emerge partially.

Usually this issue affects wisdom teeth or eighth teeth, and upper canines, with the understanding that it can affect other teeth as well.

What are included teeth and what are the causes

Included teeth represent a majoranomaly consisting of a failure of some teeth to erupt. It often happens with wisdom teeth, which are known to have no use in the dental arch. This problem can affect other dental elements as well.

Among the main causes of this abnormality is the absence of space in the alveolar bone, which consequently causes various complaints to the patient.

Dysmetabolic and genetic factors may also contribute to such a situation. However, so-called local etiological causes are more common: alteration of the eruptive sequence; basal bone alterations, etc.

The consequences of dental inclusion can be diverse and troublesome. The most common are as follows:

  • Gingivitis
  • Various pains
  • Gingival bleeding
  • Bad breath
  • Caries
  • Abscesses
  • Cysts

Cases in which one is completely asymptomatic are not uncommon, but in the long term, dental crowding can come about that must be treated in a timely manner. This is because of the possible repercussions on the oral cavity as a whole.

How we intervene to treat impacted teeth

Depending on the type of tooth inclusion, the medical team at Studio Savasta will proceed in the most appropriate manner after assessing the patient's starting picture.

As for wisdom teeth, so called because they usually appear in adulthood, the moment they are included or semi-included they must necessarily be extracted.

In fact, in most cases they create problems. Before performing such oral surgery we take an X-ray to assess the root situation.

If, on the other hand, it is the canines or other teeth, the first thing we will do is to investigate the causes, through also reconstructing the person's physiological history (e.g., the timing of eruption of other teeth). During the examination we also perform palpation to assess the presence of any lesions.

Included canines involve specific treatment due to their long roots. Their absence results in flattening of the smile and disturbances in lateral movements.

In this case we can use an orthodontic appliance to create the necessary space for the tooth. Or we can resort to orthodontic surgical salvage, by applying orthodontic traction to the included tooth element.

On the other hand, with regard to non-eruption of the upper central incisors, the reasons can be traced to the presence of benign tumors such as odontomas, or abnormal development of the dental bud.

Traumatic causes include dilaceration, that is, a bend in the root or crown, and arrested root development.

For any type of surgery we use local anesthesia, so the patient will not experience any pain. Any discomfort in the postoperative phase can be sedated thanks to anti-inflammatories.

Before we intervene, we ascertain the patient's situation using the main modern dental instruments.

One of these is orthopantomography, which allows us to see the dentition, temporomandibular joints, presence of pathologies, etc.

It allows a particularly detailed diagnosis the Cone Beam CT (Computed Tomography Cone Beam) which allows, thanks to a cone beam, to show precisely the position of the included tooth. This equipment is present at the Savasta practice.

Finally, remember that there are cases when extraction cannot be done. This is when it could result in extensive destruction of the bone or if the tooth is no longer salvageable.

Whichever of the cases described above is your case, the best solution is still to see your dentist to figure out what course of treatment to take. The visit is therefore the important time to understand the situation of your teeth.
